<br />EXHIBIT "A" <br />Scope of Services <br /> <br />b. <br /> <br />i. The notification protocol is to contact Rich Del Ben and Scott <br />Cressey forty-eight (48) hours before any treatments can occur. <br /> <br />ii. Waterworks will use the Communication Protocols identified under <br />item "G" below to contact City staff and its consultants. In the <br />event Waterworks experiences problems contacting Rich Del Ben, <br />Waterworks will contact the Public Works Services main telephone <br />number (650-780-7464) and request that messages be relayed to <br />Mr. Del Ben. <br /> <br />iii. Waterworks will anticipate the needed chemicals for the <br />applications to be made for the coming month and purchase the <br />chemicals in advance so that there are no delays to treatment due <br />to lack of supply. Billings to Redwood City will be monthly and <br />based on chemical usage / treatments. <br /> <br />iv. Survey report forms shall be as shown in Exhibit C of the <br />Waterworks proposal incorporated herein by reference. <br /> <br />If harvesting is necessary work will begin the next day and continue until <br />complete. <br /> <br />8. Waterworks staffing shall include: a boat with a four stroke motor, one Senior <br />Technician or Pest Control Operator, an additional staff technician may be <br />included to assist in launching and retrieving the boat where no ramp exists. <br /> <br />9. Waterworks is solely responsible for the administration and management of <br />safe operations by its employees at the Redwood Shores lagoon, including <br />any safety tailgate meetings, to ensure that all laws, regulation and safety <br />procedures are being followed by its staff over the term of the contract. <br /> <br />10. The surveys will be billed at the rate included in our Fee schedule. As <br />Waterworks has a yard in Redwood City, all billing time for travel is determined <br />from the Redwood City yard. <br /> <br />c. <br /> <br />REPORTING <br /> <br />1. Pre-Season Planning Meeting: Waterworks will meet with the City, between <br />March 15th and 31st of each year to review the Aquatic Plant Nuisance <br />Management Strategy to review plans for the upcoming vegetation growth <br />season. <br /> <br />2. Form A (Surveys) Report: At the end of each survey, Waterworks will make a <br />verbal report to City of Redwood City's designated staff and prepare Form A, <br />summarizing the lagoon surveys <br /> <br />3. Form B (Harvesting) and Form C (chemical applications) Reports: Forms will <br />be submitted summarizing harvesting and lor chemical applications <br />completed. Each form will be completed and sent to Redwood City by the 10th <br />day of the following month detailing all the services performed the prior month. <br />Each form should correspond with the billing charges for that month. <br /> <br />12 of 18 <br />
