<br />B. <br /> <br />APPROVAL AND INSTRUCTION NOTICE <br /> <br />The Housing and Redevelopment Division will provide the Developer the form for <br />a letter to the tenants residing in the subject property advising them of the City's <br />approval action. The Approval and Instruction Notice (Exhibit II) provides <br />eligible tenants with instructions regarding eligibility and the application process for <br />relocation assistance benefits. The responsibility of the Housing and <br />Redevelopment Director shall be to follow-up with eligible tenants residing in the <br />units at the time of application. <br /> <br />The second notice (Exhibit II), would advise the affected tenant(s) that: <br /> <br />1. <br /> <br />The p~oject was approved and demolition/conversion will occur, <br /> <br />2. <br /> <br />The projected <br /> <br />date for demolition <br />(date), <br /> <br />to <br /> <br />occur <br /> <br />IS <br /> <br />or <br /> <br />converslOn <br /> <br />3. <br /> <br />They must move by <br /> <br />4. <br /> <br />They are entitled to receive a moving assistance payment in the amount of <br />$500.00 and a Rental Assistance Payment not to exceed $1,500.00, if they <br />qualify as an eligible low or very low income household. <br /> <br />5. <br /> <br />In order to receive the assistance as described, it is necessary to complete and <br />return the attached Relocation Needs Assessment Form. The Relocation <br />Needs Assessment Form (Exhibit III)is the application for the assistance to <br />be submitted to the Housing & Redevelopment Division within 10 working <br />days of the date on which the tenant receives the notice. The assessment <br />process includes submitting supporting documentation on household income <br />to verify eligibility, attending a relocation needs assessment interview to <br />finalize the Relocation Plan, . and following defined procedures to receive <br />payment. <br /> <br />6. <br /> <br />They must begin searching for a replacement housing unit and notify the <br />Housing and Redevelopment Division designee when they have located the <br />replacement unit. They must submit a copy of rental or lease agreement to <br />the Housing & Redevelopment Division. <br /> <br />The Approval and'Instruction Notice for Non-Residential projects will provide <br />the application for tenants to apply for both the moving expense payment of <br /> <br />4 <br /> <br />
