Ezhibit A Page A- 6 <br /> PROGRAM DESCRIPTION <br /> HSA staff person makes the decision, in consultation if necessary with their HSA <br /> supervisor. Where HSA and Core are jointly working with a client and it is an issue <br /> involving interpretation of regulations, then HSA staff will make the decision in <br /> consultation if necessary with their HSA supervisor or specialist. If the issue where <br /> jointly working does not involve a regulation, then the Core Supervisor may make the <br /> decision in the moment. <br /> B. Intake <br /> When a client cQmes into Contractor's office,they will be given the three page Screening <br /> and Assessment Tool (C-250). After addressing any emergency needs, if Contractor <br /> identifies a client as being eligible for TANF or other HSA services, that client will be <br /> refened to HSA staff for further assistance, including use of the Broad Based Evaluation <br /> form. If further Core services are needed, HSA staff will refer the client back to <br /> Contractor. If the clierrt does not appear eligible for HSA services,then the client remains <br /> with Contractor for services. <br /> C. Training <br /> All training on the SUCCESS system will be available to Contractor staff. The grant from <br /> the David and Lucille Packard Foundation will provide training in change management <br /> for Contractor and Board members and will allow the HSA to share its training with <br /> Contractor. <br /> D. Family Self-Sut�iciency Teams (FSST) <br /> Contractor will continue to be participants in the FSST. Contractor staffmay be primary <br /> case managers for FSST's, where they have the requisite training and experience. <br /> E. FinanciaVPotential Modifications <br /> HSA will evaluate the need for increased funding for Contractor and the other Core <br /> Agencies and advocate adjustments as appropriate to their contract. <br /> HSA will pay Contractor for reasonable overhead costs for out-posted HSA staff, as <br /> staffing permits, at Contractor's office, including rent, at a rate to be determined. <br /> Quarterly,HSA and Contractor will evaluate changes in the volume of clients seeking and <br /> receiving services with Contractor, determining if volume increase is due to welfare <br /> reform, or due to the screening and assessment process. If changes in staffing or <br /> resources are indicated, HSA will seek to respond in the next fiscal year or sooner if <br /> possible. If extreme variations occur during contract period, non-budgetary adjustments <br /> may be agreed to. <br /> C:\CONTRACTI FYOOCON\10FOCCOO.AGT <br />
