�. 3�c ic. � <br /> Exhibit A Page A- 4 <br /> PROGRAM DESCRIPTION <br /> Outreach, Marketing and Pubticity: The program is advertised on a quarterly basis in the <br /> Citywide newsletter which is sent to all Redwood City residences. Monthly reports are <br /> submitted and reviewed by the Department Head, Fair Oaks Community Center Advisory <br /> Board, and the City Parks and Recreation Commission. The primary means of outreach and <br /> publicity for the program has been and will continue to be word of mouth. <br /> IV. DESCRIPTIONS OF THE SERVICE ENVIRONMENTS <br /> Clients Served: The program serves low income residents, 90% of which are between the <br /> ages 22 and 54, 70% non-white and predominantly low income. Over 50% of these clients .. <br /> are Hispanic, with most of this group being very limited in the ability to speak English. <br /> Translation and forms assistance and emergency food, clothing and lodging are the most <br /> requested services by clients of the program. <br /> Contract Services Staflf: Two and one-half, full-time equivalent employees are allocated to <br /> this program: <br /> Human Service Specialist UII (1.8 FTE) <br /> Human Service Specialist III (1.0 FTE) <br /> Volunteers: Currently two volunteers are donating 4-S hours per week to assist clients in <br /> completing forms and doing translations. <br /> Board of Directors: The City Council of the City of Redwood City acts as the Board of <br /> Directors. <br /> V. OTHER CONTRACTOR RESPONSIBILITIES <br /> A. The contractor will provide a system through which recipients of service shall have the <br /> opportunity to express and have considered their views, grievances, and complaints <br /> regarding the delivery of services. <br /> B. The contractor agrees to keep the San Mateo County Human Services Agency informed <br /> about its services and activities under this contract, and to accept appropriately referred <br /> clients from the San Mateo Counry Human Services Agency for its contract services as <br /> part of its client base. <br /> C. The contractor agrees to cooperate with the State Department of Social Services (SDSS) <br /> and the San Mateo County Human Services Agency on the implementation, monitoring, <br /> and evaluazion of this project and to comply with any and a11 reporting and evaluation <br /> requirements established by SDSS. <br /> D. The contractor shall establish a procedure to ensure that all employees, volunteers, <br /> consultants, subcontractors, or agents perfornung services under this contract report child ' <br /> abuse or neglect to a child protective agency defined in Penal Code Section 11165(k). <br /> F:\CONTRACTl1 fy01 con110focc0l.wpd <br />
