Types of Attachments <br />• None <br />• Agreements <br />• Resolutions <br />• Ordinances <br />• Map <br />• Other <br />• Related Documents Located in the City Clerk's Office <br />n. Mass Mailer Sent Out? (yes/no); <br />o. Is this a Public Hearing (yes/no); <br />p. Was the Public Hearing Notice published? (yes/no); <br />q. Is this a Successor Agency Item? (yes/no); <br />r. Is this a Staff Report? (yes/no); <br />s. Is this a Consent Calendar Item? (yes/no); <br />t. Is this an Informational Item? (yes/no); <br />u. Is this Time Sensitive? (yes/no); <br />Hyland will configure a Word Document Template Summary Sheet that will be generated. by <br />OnBase Agenda and populated from the above form information. <br />Upon completion of the form the Agenda Item Approval workflow will be started. This <br />workflow will contain the following steps for item approval. <br />a. Submitter Step: <br />1. Will receive an email notification when item arrives. <br />2. Workflow queue based on the user who created the Agenda Item; <br />3. Will be able to review/edit the agenda item form; <br />4. Will be able to add/edit/revise attachments to the agenda item form; <br />5. Will be able to create/participate in Discussion threads; and <br />6. Will have the following routing options (Ad Hoc Tasks): <br />L Send to Department Head Approver; <br />ii. Send to an Additional Reviewer; and <br />iii.
