6.1.A. - Page 20 <br />Succinctly describe the proposed approach for addressing the required services <br />requested in Attachment A (Scope of Services). <br />Additionally, provide details about your firm's: <br />• Compliance with the California Injury and Illness Prevention Program (SB 198). <br />• Compliance with prevailing wage guidelines, including the posting of these <br />guidelines on jobsite notices. <br />• Safety training: describe your training program, training resources utilized, <br />initial training plan and continuing training plan. <br />• Shop, storage, equipment, and vehicle inventory. <br />Chapter 5 — Project Staffing <br />This Chapter shall discuss how the Proposer will staff this project. Key project team <br />members shall be identified by name, title, and specific responsibility for the project. <br />Resumes for all of the Proposer's personnel shall be included. <br />Contractors will provide proof of a valid, current, and in good standing relevant <br />Contractor's license issued by the California State Contractor Licensing Board. <br />Chapter 6 — Proposal Cost <br />Provide a cost breakdown in an easily readable format. All pricing must be inclusive, <br />and include labor, material, and equipment necessary for each task listed in <br />Attachment A (Scope of Services). Provision of this information assists the City in <br />determining whether the Proposer understands the project, whether the costs are <br />fair and reasonable in light of the services to be provided, and provides City staff with <br />tools to negotiate the final cost. <br />The Proposer shall be responsible for all costs incurred in the development and <br />submission of this response. The City assumes no contractual obligation as a result of <br />the issuance of this RFP, the preparation or submission of a response by a Proposer, <br />the evaluation of an accepted response, or the selection of finalists. The City shall not <br />be contractually bound until the City and the successful Proposer have executed a <br />written contract for performance of the work. The City of Redwood City does not pay <br />for services before it receives them. Submitted Proposals should not propose contract <br />terms that require upfront payment or deposits. <br />Chapter 7 — Client References <br />In a format similar to Attachment B, supply a minimum of two to three references <br />which are past and current clients. Additional references may be included which <br />reflect a similar scope of work to this project.
