Exhibit A Page A - 4 PROGRAM DESCRIPTION Outreach, Marketing and Publicity: The program is advertised on a quarterly basis in the Citywide newsletter which is sent to all Redwood City residences. Monthly reports are submitted and reviewed by the Department Head, Fair Oaks Community Center Advisory Board, and the City Parks and Recreation Commission. The primary means of outreach and publicity for the program has been and will continue to be word of mouth. IV. DESCRIPTIONS OF THE SERVICE ENVIRONMENTS Clients Served: The program serves low income residents, 90% of which are between the ages 22 and 54, 70% non-white and predominantly low income. Over 50% of these clients are Hispanic, with most of this group being very limited in the ability to speak English. Translation and fOnDS assistance and emergency food, clothing and lodging are the most requested services by clients of the program. Contract Services Staff: Two and one-half, full-time equivalent employees are allocated to this program: Human Service Specialist I/II (1.8 FTE) Human Service Specialist ill (1.0 FTE) Volunteers: Currently two volunteers are donating 4-8 hours per week to assist clients in completing fonns and doing translations. Board of Directors: The City Council of the City of Redwood City acts as the Board of Directors. V. OTHER CONTRACTOR RESPONSmILITIES A. The contractor will provide a system through which recipients of service shall have the opportunity to express and have considered their views, grievances, and complaints regarding the delivery of services. B. The contractor agrees to keep the San Mateo County Human Services Agency infonned about its services and activities under this contract, and to accept appropriately referred clients from the San Mateo County Human Services Agency for its contract services as part of its client base. C. The contractor agrees to cooperate with the State Department of Social Services (SDSS) and the San Mateo County Human Services Agency on the implementation, monitoring, and evaluation of this project and to comply with any and all reporting and evaluation requirements established by SDSS. D. The contractor shall establish a procedure to ensure that all employees, volunteers, consultants, subcontractors, or agents performing services under this contract report child abuse or neglect to a child protective agency defined in Penal Code Section I I I 65(k).
