<br />The Department may require any and all persons who are <br /> <br />registered with the Department pursuant to this article to <br /> <br />file with the Health Officer at any time and at such frequency <br /> <br />or intervals as is desirable, a statement giving the name and <br /> <br />address of the owner or tenant of each and everyone of the <br /> <br />premises where a chemical toilet shall have been cleaned out <br /> <br />by such registrant or his employees or by others on his behalf <br /> <br />and such statement shall also describe in precise terms the <br /> <br />place where the cleanings shall have been disposed of and <br /> <br />by whom. <br /> <br />The Department may require such statements to be <br /> <br />sworn to before a notary. <br /> <br />Change of Address. <br /> <br />A change of business <br /> <br />"Section 27.110. <br /> <br />or residence address of any registrant under this article, <br /> <br />including a member of a partnership which is registered, <br /> <br />shall be reported in writing by registered mail by the regis- <br /> <br />trant within two days after such change of address. <br /> <br />"Section 2 7 . Ill. <br /> <br />Revocation. <br /> <br />Any registration issued <br /> <br />under this article may be revoked by the Department for cause <br /> <br />on ten days' notice to the applicant, which notice shall be <br /> <br />served by registered mail or in person at the latest place <br /> <br />of residence or of business reported by the applicant. <br /> <br />"Section 27.112. <br /> <br />Rules and Regulations. <br /> <br />The <br /> <br />Department is authorized to issue rules and regulations <br /> <br />in order to administer and carry out the purpose of this <br /> <br />article. <br /> <br />"Section 27.113. <br /> <br />Violations Unlawful. <br /> <br />It shall be <br /> <br />unlawful to violate any of the provisions of this article <br /> <br />or any order of the Department made pursuant to this <br /> <br />article for the protection of human health and comfort. <br /> <br />"ARTICLE VIII. <br /> <br />"OBSTRUCTIONS TO STORM WATER DRAINAGE. <br /> <br />-39- <br />
