Contractor will be responsible for submitting 6-month and year-end narrative reports and year- <br />end financial reports. <br /> <br />6-Month reports are to be submitted as follows: <br />January 31,2007 <br />January 31, 2008 <br /> <br />Year-end reports and year-end financial reports are to be submitted as follows: <br />July 31,2006 <br />--~--- ---- <br />July 31,2007 <br />July 31,2008 <br /> <br />Reports will be submitted to: Marissa Saludes, Contract Monitor, Human Services Agency, 400 <br />Harbor Blvd., Bldg. B, Belmont, CA 94002. <br /> <br />2. Contractor will submit to the Children and Family Services Contract Monitor at 400 Harbor Blvd. Bldg. B., <br />Belmont, CA 94002, a roster of RWC 2020's Board of Directors and meeting dates with the year-end report. <br /> <br />3. Contractor will submit to the Children and Family Services Contract Monitor at 400 Harbor Blvd. Bldg. 8., <br />Belmont, CA 94002, a financial audit, as soon as it becomes available. <br /> <br />4. County will conduct site visit during the term of the Agreement to review all aspects of program operations <br />and review Contractor's documentation related to case management. This site visit will be arranged in <br />advance with the Director of RWC2020. <br /> <br />5.
