J. Provide formal feedback to your Member on his or hwr p erforanance at least twice during the <br />program }ear. You must use Public Allies performance evaluation tools and are free to use <br />any other tools your organization may use. These Member Performance assessments are due <br />at the mid -point and at the end of the program year. <br />F. Attend your Members end oftheyear presentation and graduation scheduled for either June <br />20,21, 24 or 25. <br />2. Member Eligibility <br />In order to be placed, Public Allies Members must meet the following criteria: <br />A. Is at least 17 yens of age by the start dale; <br />B. Is a D.S. citizen or D.S. national or lawful permanent resident alien of the ITnited States; <br />C. Has a high school diploma or equivalent; <br />f). Has a complete application on file (either online and noted as such in file; ora print copy is <br />in tile), <br />E. Satisfactorily completer] a previous term ofsereice if there is tare; and <br />P. Passes a National Service criminal history check. <br />Asa part of the National Service Criminal History check, Public Allies conducts a search of the <br />National Se. Offender Public Website (NSOPW), statewide criminal history search, and/or FBI <br />fingerprint check. Each of these checks must be initiated and the NSOP& results must be verified <br />and documented prior to the Ally beginning his/her term of service. Any additional criminal history <br />check your agency chooses to perform is at its own cost. Public Allies cannot share the results of its <br />criminal history checks with your agency, but Ave can provide a letter stating that the Ally is eligible <br />and qualified forthe program. <br />An Ally who has passed the NSOPW and awaiting trsulls of the state and PBI checks may begin <br />service, so long as they are physically accompanied while working with individuals fiom a vulnerable <br />population, which is defined as someone under the age of 18, over the age of 60, or having a <br />disability. Physical accompaniment can be provided by: <br />• An authorized representative who has previously been cleared for such access; <br />• Afamily member or legal guardian of the vulnerable individual; <br />• An individual authorized by nature of his/her profession to have recurring access to the <br />vulnerable individual, such as an education or medical professional. <br />If an Ally is required to be accompanied while in service with your agency', you mast document the <br />dates, limes, names and signatures of the individuals providing accompaniment. <br />In carrying out the Program, Public Allies or the Partner Organisation shall not discriminate against <br />any Member or Program applicant because of race, color, religion, sex, sexual orientation, national <br />origin, disability, age, political affiliation, or any other characteristic protected by local, State or <br />Federal law. It is also unlawful to retaliate against any person who, or organization that, files a <br />complaint about such discrimination. In addition to filing a complaint with local and state agencies <br />that are responsible for resolving discrimination complaints, you may bring a complaint to the <br />attention of the Corporation for National and Community Service.
