6.13. - Page 91 of 366 <br />17.5.3 If the benefits provided under the federal or state act exceed the benefits provided <br />hereunder in each category of coverage, the City shall be under no further <br />obligation to make any contribution. <br />17.6 Life Insurance <br />The City shall offer to eligible employees additional life insurance equal to one and one-half <br />(1-112) times the employee's annual salary at a 60140 premium contribution split between <br />the City and the employee respectively. <br />17.7 Long Term Disability <br />The City will contract to provide Long Term Disability Insurance for COA unit employees. <br />The City, for the term of this agreement, will pay the full cost of the basic rate (basic rate <br />provides for up to a three thousand dollar ($3,0001 maximum monthly payout). A buy -up <br />option will be included to offer the employee the opportunity to increase their coverage, at <br />their own expense, up to an additional three thousand dollar {$3,0001 monthly payout. The <br />total maximum monthly payout available will be six thousand dollars J$6,0001. <br />17.8 Social Security <br />In the event the City and its employees are required to participate in the Federal Social <br />Security Program, the contribution designated by law to be the responsibility of the <br />employee shall be paid in full by the employee and the City shall not be obligated to pay or <br />' pick up" any portion thereof. <br />17.9 COBRA <br />The City may cause employees not entitled to the benefits set forth in Article 17, who are <br />allowed to remain on a City health insurance plan following separation from employment <br />pursuant to the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), to be <br />charged for such coverage at the maximum rate permissible by law (presently 102% of the <br />premium for an active employee). <br />ARTICLE 18 <br />SAFETY <br />18.1 Both the City and the Association shall expend every effort to ensure that work is performed <br />with a maximum degree of safety, consistent with the requirement to conduct efficient <br />operations. <br />18.2 Each employee covered by this Memorandum agrees to comply with all safety rules and <br />regulations in effect and any subsequent rules and regulations that may be adopted. <br />Employees further agree that they will report all accidents and safety hazards to the <br />appropriate management official immediately. Any employee having knowledge of or who is <br />witness to an accident shall, if requested, give full and truthful testimony as to same. <br />18.3 Employees shall not be required to operate equipment that has been deemed unsafe. <br />42 <br />110 <br />
