6.K. - Page 65 of 190 <br />18.8 Pregnancy Leave of Absence Without Pay <br />18.8.1 Pregnancy leave of absence without pay or benefits shall be <br />granted upon request to led- uali in . _ probationary and <br />permanent -employees for that period of time requested, up <br />to one (1) year. The combination of pregnancy leave and parental <br />leave will not exceed one (1) year. <br />18.8.2 Pregnancy leave shall be granted when the following conditions <br />have been met: <br />18.8.2.1 An employee who is pregnant and seeks leave under <br />this provision shall report her condition, <br />to her <br />immediate supervisor, the Shift Battalion Chief or <br />Personnel Officer. <br />18.8.2.2 At least thirty (30) days prior to the beginning of the <br />pregnancy leave, the employee shall submit to the <br />Personnel Officer the specific date she intends to <br />begin the leave, accompanied by her physician's <br />written statement attesting to the employee's ability <br />to continue performing the full schedule of her duties <br />and responsibilities. She shall continue on active <br />duty until the specific date, providing she performs <br />the full duties and responsibilities of her position and <br />furnishes additional health statements from her <br />physician upon reasonable request. In the event the <br />employee is unable to perform the fug --essential <br />duties and responsibilities of her position, she shall <br />be assigned to light duty until the specific leave date, <br />and shall continue to furnish additional health <br />statements from her physician upon reasonable <br />request. <br />18.8.2.3 Prior to the establishment of a specific date for <br />return to duty, the employee shall submit to the <br />Personnel Officer a notice of intention to return to <br />duty, accompanied by her physician's statement <br />certifying that the employee is medically qualified to <br />assume regular duties and responsibilities. <br />18.8.2.4 The Personnel Officer or his/her designee may <br />designate the specific beginning and ending dates <br />within the pay period requested by the employee to <br />meet the needs of the employee and the City. <br />54 <br />308 <br />
