4/3/00 <br />The Approval and Instruction Notice would advise the affected tenant(s) that: <br />1. The project was approved and demolition /conversion will occur, <br />2. The projected date for demolition or conversion to occur is <br />(date), <br />3. They must move out by <br />4. They are entitled to receive a moving assistance payment in the amount of <br />$500.00 and a Rental Assistance Payment not to exceed $1,500.00, if the qualify as an <br />eligible low income household. <br />S. In order to receive the assistance as described therein, it is necessary to <br />complete and return the attached Relocation Needs Assessment Form (Exhibit III). <br />The Relocation Needs Assessment Form is the application for the assistance to be <br />submitted to the Planning & Redevelopment Division within 10 working days of the date <br />on which the tenant receives the notice. The assessment process includes submitting <br />supporting documentation on household income to verify eligibility, attending a <br />relocation needs assessment interview to finalize the Relocation Plan, and following <br />defined procedures to receive payment. <br />6. Begin searching for a replacement housing unit and notify the Planning and <br />Redevelopment Division designee when you have located the replacement unit. <br />Submit copy of rental or lease agreement to the Planning & Redevelopment Division. <br />The Approval and Instruction Notice for Non - Residential projects will provide <br />the application for tenants to apply for both the moving expense payment of $500.00 <br />and the Relocation Assistance Payment. The Director, or his or her designee, may <br />provide tenants with vacant rental information as it is available, however, the primary <br />responsibility for locating the replacement unit shall be the tenants. <br />C. • METHOD OF DISBURSEMENT <br />Eligible tenants must complete a Relocation Claim Form (Exhibit IV) which <br />provides information on replacement housing costs at their new unit, the Manager will <br />calculate the Housing Assistance payment due by subtracting the old rent plus utilities <br />from the new rent plus utilities for 12 months and making a payment to the tenant in <br />that amount, not to exceed a one time payment of $1,500. <br />At the time the development application is approved, the Developer will deposit <br />vc /C C11 /egmt <br />Agmt -3 42 <br />FXS /£B <br />03/]9/04 <br />7 <br />13861 <br />
