6.1 F <br /> Page 25 <br /> to furnish additional heatth statements from her physician <br /> upon reasonable request. <br /> 9.6.2.3 Prior to the establishment of a specific date for return to <br /> duty, the emptoyee shall submit to the Personnel Officer a <br /> notice of intention to return to duty, accompanied by her <br /> physician's statement certifying that the employee is <br /> medically qualified to assume regular duties and <br /> responsibitities. <br /> 9.b.2.4 The Personnet Officer ar his/her designee may desi�nate <br /> the specific beginnin� and ending dates within the pay <br /> period requesCed by the employee to meet the needs of the <br /> employee and the City. <br /> 9.6.3 The empioyee on leave shall be returned to an equivatent posifiion <br /> within her ctassification. <br /> 9.6.4 A maternity teave, absent physical disability, is granted without pay <br /> for the duration of the leave. The employee may elect to continue <br /> medicat and dental insurance coverage for up to one (1) year during <br /> that [eave at her own expense. <br /> 9.7 Leave for Pregnancy Disability <br /> 9.7.1 Employees who are working are entitted to use personal illness and <br /> injury leave for disabilities caused or contributecf ta by pregnancy, <br /> miscarriage, childbirfih, and recavery therefrom on the same terms <br /> and conditions governing leaves of absence for other illness or medicat <br /> disability. Such leave shall not be used for child care, chitd rearing, <br /> or preparation for childbearing, but shalt be limited to those <br /> disabilities as set forth above. The length of such disability leave, <br /> including the date on which the leave shall commence and the date on <br /> which the duties are to be resumed, shall be determined by the <br /> employee and the employee's physician; however, the Personnel <br /> Officer may require a verification of the extent of disability through a <br /> physical examination of the employee by a physician appointed by the <br /> City at City expense. <br /> 9.7.2 Emptoyees are entitled to leave without pay or other benefits for <br /> disabflities because of pregnancy, miscarriage, childbirth, or recovery <br /> therefrom when sick teave has been exhausted. The date on which <br /> the employee shall resume duties shall be determined by the <br /> emptoyee on leave and the employee's physician; however, the <br /> Personnet Officer may require a verification of the extent of disability <br /> 22 <br />
