6.1 F <br /> Page 27 <br /> 9.1d.7.1 to care for a child following a birth or placement of a child <br /> with the emptoyee for adoption or foster care; <br /> 9.10.1.2 to care for the spouse, chitd or parent of the employee <br /> who has a serious health conditian; <br /> 9.10.1.3 if the employee is unabte to perform his/her job because <br /> of the emptoyee's own serious heatth condition. <br /> 9.10.2 When an employee desires Ieave for the above stated reasons, the <br /> ernp[oyee should give 34 calendar days written advance notice to <br /> his/her immediate supervisor (ar if the event is unforseeable <br /> employee shouEd give as much notice as possible). <br /> 7he City may require that a request for family Eeave be supported by <br /> a certification issued by the employee's health care provider. <br /> Certification may also be required from the health care provider of <br /> the emptoyee's child, parent or spouse who has a serious health <br /> conditit�n before the employee is approved ta use family/medical <br /> teave to take care of that famity member. <br /> Failure to comply with the above requirements may tead to denial of <br /> leave. <br /> 9.10.3 The emp[oyee may elect, or the City may require the employee, to <br /> substitute accrued vacation leave or other accrued time off durin� <br /> family/medicat leave. Sick leave may be used during the period of <br /> family/medical leave in accordance with the sick leave provisions <br /> found in Section 9.1 of this Memorandum of Understanding. <br /> 9.10.4 The City witt maintain contributions for the employee's heatth benefits <br /> under group health plans at the same level as prior ta the leave, in <br /> accordance with the Family &r Medical Leave Act. The equivalent <br /> benefits, pay and other terms and conditions of emplayment upon <br /> completion of the leave. <br /> SECTION 10.
