REV: 06-14-23 RL <br />C. Collaboration with Partner Agencies <br />Consultant will work closely with partner agencies including Redwood City Outreach <br />Strategy Team, LifeMoves, WeHOPE, Street Life Ministries, Nation’s Finest, <br />Redwood City Police Department, Fair Oaks Community Center, Mental Health <br />Association of San Mateo County, and other key partner agencies as identified by <br />the Homeless Services Manager or designee to assure services to individual Team <br />Members are coordinated. <br />D. Coordination with City of Redwood City <br />Consultant will coordinate with the City of Redwood City on project implementation <br />and performance outcomes, including, but not limited to: <br />Monthly check-ins with the Community Policing Officer, Homeless Services <br />Manager, Public Works Right of Way Superintendent, and other City staff <br />designated by the Homeless Services Manager for the purpose of <br />coordinating, reporting and monitoring Consultant’s efforts to maximize <br />impact; <br />Participation in the monthly Homeless Outreach Team Multi-Disciplinary <br />Team (MDT) meetings; and <br />Participation in the Redwood City Homeless Outreach and Service <br />Provider’s group; and <br />Participation in other meetings as directed by the Homeless Services <br />Manager. <br />E. Staffing <br />Consultant will build up to a standing Team of at least 20 individuals and serve 60 <br />individuals as Team Members in the contract year. Consultant's staffing will be <br />comprised of one Program Director, one Project Manager, two Case Managers, and <br />three Drivers. <br />The Program Director will be responsible for supervising the Project Manager, <br />providing overall City-wide program management, budgeting, fundraising, <br />staff support, and programmatic vision. <br />The Project Manager will manage Consultant staff and Team Members, as <br />well as all operations, facilitating the weekly Team meeting and be <br />responsible for all community relations. The Project Manager will act as the <br />direct contact for Redwood City and all project partners. <br />The Case Managers will provide resources to Team Members, provide one- <br />on-one case management sessions, provide employment services, advocate <br />for Team Members and build partnerships with service providers, employers <br />and landlords to place Team Members into housing and employment. <br />ATTY/AGR.2023/Amend.
