REV: 05-22-24 LF <br />C. Collaboration with Partner Agencies <br />Contractor will work closely with partner agencies including Redwood City <br />Outreach Strategy Team, LifeMoves, WeHOPE, Street Life Ministries, <br />Nation’s Finest, Redwood City Police Department, Fair Oaks Community <br />Center, Mental Health Association of San Mateo County, and other key <br />partner agencies as identified by the Homeless Services Manager or <br />designee to assure services to individual Team Members are coordinated. <br />D. Coordination with City of Redwood City <br />Contractor will coordinate with the City of Redwood City on project <br />implementation and performance outcomes, including, but not limited to: <br />Monthly check-ins with the Community Policing Officer, Homeless <br />Services Manager, Public Works Right of Way Superintendent, and <br />other City staff designated by the Homeless Services Manager for <br />the purpose of coordinating, reporting and monitoring Contractor’s <br />efforts to maximize impact; <br />Participation in the monthly Homeless Outreach Team Multi- <br />Disciplinary Team (MDT) meetings; and <br />Participation in the Redwood City Homeless Outreach and Service <br />Provider’s group; and <br />Participation in other meetings as directed by the Homeless Services <br />Manager. <br />E. Staffing <br />Contractor will build up to a standing Team of at least 15 individuals and <br />serve 45 individuals as Team Members in the contract year. Contractor's <br />staffing will be comprised of up to one Program Director, one Project Manager, <br />two Case Managers, and three Drivers. <br />The Program Director will be responsible for supervising the Project <br />Manager, providing overall City-wide program management, <br />budgeting, fundraising, staff support, and programmatic vision. <br />The Project Manager will manage Contractor staff and Team <br />Members, as well as all operations, facilitating the weekly Team <br />meeting and be responsible for all community relations. The Project <br />Manager will act as the direct contact for Redwood City and all <br />project partners. <br />The Case Managers will provide resources to Team Members, <br />provide one-on-one case management sessions, provide <br />employment services, advocate for Team members, and build <br />ATTY/AGR.2024/Amend.
