Contractor will meet at least once during each fiscal year of this Agreement with the City <br />Manager and Human Services Manager for a performance review and will subsequently provide <br />an update presentation at a regularly scheduled City Council meeting. <br />Contractor will build up to a standing Team of at least 20 individuals and serve 80 individuals in <br />the first year. Contractor's staffing will be comprised of three full-time individuals. A Project <br />Manager will manage Contractor staff and Team Members, as well as all operations, fundraising <br />efforts, facilitating the weekly Team meeting and be responsible for all community relations. The <br />Project Manager will act as the direct contact for Redwood City and all project partners. A Case <br />Manager and Employment Specialist will provide resources to Team Members, provide one-on- <br />one case management sessions, provide employment services, advocate for Team Members and <br />build partnerships with service providers, employer and landlord to place Team Members. <br />Contractor is committed to reporting on the following outcomes on a monthly basis: <br />§ Team size and individuals served <br />§ Team Members graduated into employment <br />§ Team Members moved into permanent housing <br />§ Debris removed in cubic feet, including needles removed (Police/Public Works will <br />partner on training and removal) <br />§ Volunteer hours served <br />§ Team Member survey results collected annually reflecting self-esteem, motivation, and <br />hope as well as positive outcomes re health, mental health, substance abuse treatment, <br />criminal justice system interactions <br />2. Payment Terms. <br />The City agrees to pay Contractor a total amount that shall not exceed Three Hundred Seventy - <br />Eight Thousand Six Hundred Sixty -Four Dollars ($378,664) each year of the two-year pilot <br />program for a total not to exceed amount of Seven Hundred Fifty -Seven Thousand Three <br />Hundred Twenty -Eight Dollars ($757,328), which amount shall include all expenses. <br />An initial payment of 20% of the first year's cost, totaling Seventy -Five Thousand Seven <br />Hundred Thirty -Three Dollars ($75,733), will be paid to Contractor upon execution of this <br />Agreement. <br />Contractor shall submit bills on a monthly basis together with monthly progress reports to the <br />Human Services Manager. Contractor's bill shall include the following information for the <br />project: a brief description of services performed, the date the services were performed, the <br />number of hours spent and by whom, a brief description of any costs incurred, percent <br />completion to date of each task and subtask, total of prior billings, amount for billing period, <br />total billing to date.
