6.1). - Page 27 of 38 <br />Attachment B — Cost Proposal Schedule <br />Part 1 <br />Proposers shall provide a comprehensive cost proposal schedule in Chapter 5 — Proposal Cost. <br />Proposer should include: <br />• hourly labor rate by class, including straight time and overtime rates (if applicable), <br />• equipment rates by class, including straight time and overtime rates (if applicable), <br />• material markup, <br />• annual preventative maintenance cost (flat fee, should include all labor, equipment, <br />parts, and associated costs), <br />• estimated annual cost for parts and materials, <br />• estimated cost range for major repairs by repair type, <br />• rate for adding/deleting pumps subject to the agreement. <br />All related costs to complete the work set forth in Attachment A — Scope of Services should be <br />specified. <br />Part 2 <br />For the purpose of evaluating proposals and in addition to the above, Proposer shall provide <br />estimates for the repair and replacement of specific pumps and motors as follows. Pumps and <br />motors will not be removed for this purpose. Should additional information be required, <br />Proposers shall submit questions as outlined in section 5.5 of this RFP. Additional information will <br />be available during the mandatory site visit, and Proposers will be able to view, evaluate and take <br />measurements for each pump and motor. Some assumptions may need to be made to complete <br />these estimates.
