Any affected areas of paving and <br />King Street <br />P1-65 <br />SFPUC Engineering <br />July, 2009 <br />STA 319+25 <br />sidewalks shall be restored per Redwood City standards. <br />Management Bureau <br />CCSF's Contractor shall conduct closed circuit television (CCTV) <br />inspection of the 6 -inch existing sewer pipe which crosses the new <br />proposed pipeline in King Street. <br />CCSF's Contractor shall restore any affected areas of Red Morton <br />Park, including paving, landscaping, and sports facilities. All <br />construction activities within the park, including restoration, shall <br />Red Morton Park <br />P1-659 P1-66 <br />SFPUC Engineering <br />July, 2009 <br />STA 319+50 to <br />occur between June 15 and September 15 of either 2010 or 2011. <br />C34-1 <br />Management Bureau <br />STA 328+80 <br />Contractor shall be allowed to use the paved parking lot at the end <br />of Myrtle Street as a temporary staging area between June <br />15 and September 15 of either 2010 or 2011 (but not both years). <br />Pipeline installation shall occur between 7:30AM to 4:30PM, M -F. <br />CCSF's Contractor shall maintain one 11 ft. reversible lane during <br />work hours. Any affected areas of paving and sidewalks shall be <br />restored per Redwood City standards. CCSF shall submit 2 <br />settlement monitoring reports on the adjacent creek. The first <br />report shall be submitted at the end of the first month after the <br />Myrtle Street <br />P1-66 <br />SFPUC Engineering <br />July, 2009 <br />STA 325+50 <br />completion of the trenchless work. The second report shall be <br />Management Bureau <br />submitted at the 11th month after the completion of the trenchless <br />work and, prior to the expiration of the 12 -month warranty <br />period. Temporary survey benchmarks shall be established for the <br />monitoring to assure no settlement has occurred. Contractor shall <br />conduct closed circuit television (CCTV) inspection of the 6 -inch <br />existing sewer pipe which crosses the new proposed pipeline in <br />Myrtle Street. <br />Pipeline installation shall occur between 7:30AM to 4:30PM, M -F, <br />when the road may be closed.. Any affected areas of paving and <br />Madison Avenue <br />P1-67 <br />SFPUC Engineering <br />July, 2009 <br />STA 329+10 <br />sidewalks shall be restored per Redwood City standards. <br />Management Bureau <br />CCSF's Contractor shall conduct closed circuit television (CCTV) <br />inspection of 15 -inch existing sewer pipe which crosses the new <br />proposed pipeline in Madison Avenue. <br />Pipeline installation shall occur between 7:30AM to 4:30PM, M -F, <br />when the road may be closed. Any affected areas of paving and <br />Hawes Street <br />P1-67 <br />SFPUC Engineering <br />July, 2009 <br />STA 330+30 <br />sidewalks shall be restored per Redwood City standards. <br />Management Bureau <br />CCSF's Contractor shall conduct closed circuit television (CCTV) <br />inspection of 6 -inch existing sewer pipe which crosses the new <br />proposed pipeline in Hawes Street. <br />All construction, including restoration, shall occur during a single <br />season, between June 15 and August 15 only. Open cut <br />construction. CCSF's Contractor shall maintain one 11 ft. lane in <br />each direction open during work hours, 9:OOAM to 4:OOPM, and <br />four lanes open at all other times. Contractor shall place <br />Jefferson Avenue <br />P1-68 <br />SFPUC Engineering <br />July, 2009 <br />STA 333+20 <br />changeable message signs in both traffic directions advising drivers <br />Management Bureau <br />of lane closure schedule for at least one week in advance of and <br />continuing during work.
