<br />EXHIBIT A- DESCRIPTION OF SERVICE <br /> <br />The following services will be provided by the Fair Oaks Senior <br />Center (TEWCI) during this contract period: <br /> <br />Information and Referral services will be provided in accordance <br />with the California Department of Aging standards and Area Agency <br />on Aging policy. <br /> <br />A minimum of 215 unduplicated seniors will be served for a total <br />of: <br /> <br />339 Information units of service <br />170 Referral units of service <br />56 Evaluation/Follow-up units of service <br />136 Outreach units of service <br />1,694 Comprehensive Assessment units of service <br />(serving a minimum of 65 clients) <br /> <br />Senior Center Management will be provided as defined in the <br />Summary of Program Requirements, with no units of service <br />provided. <br /> <br />Transportation services will be provided as defined in the Summary <br />of Program Requirements and the AAA Transportation Policy. A <br />minimum of 100 unduplicated seniors will be served for a total of <br />9,217 units of service. <br /> <br />Management of a Congregate Nutrition Site will be provided as <br />defined by the California Department of Aging and the Area Agency <br />on Aging policy. A minimum of 350 unduplicated seniors will be <br />served for a total of 18,000 meals served. Nutrition Education <br />will be provided for a minimum of 12 units of service. The Area <br />Agency on Aging will pay for the cost of meals served under <br />separate contract. Project income collected for this portion of <br />the program will be submitted to the Area Agency on Aging with the <br />Monthly Expenditure Report. <br /> <br />The following services will be provided by the Veterans Memorial <br />Senior Center during this contract period: <br /> <br />Information & Referral services will be provided as defined in the <br />Summary of Program Requirements and in accordance with the <br />California Department of Aging standards and the Area Agency on <br />Aging policy. <br /> <br />A minimum of 165 unduplicated seniors will be served for a total <br />of: <br /> <br />245 Information units of service <br />123 Referral units of service <br />41 Evaluation/Follow-up units of service <br />104 Outreach units of service <br />1,241 Comprehensive Assessment units of service <br />(serving a minimum of 70 cases) <br /> <br />Senior Center Management will be provided as defined in the <br />Summary of Program Requirements, with no units of service <br />provided. <br /> <br />A coordinated Transportation program, serving two sites- the <br />Veterans Memorial Senior Center, and the Fair Oaks Senior Center <br /> <br />-7- <br />
