REQUIRED PROPOSAL SUBMISSION DOCUMENTS <br />As a part of their proposal submission, Proposers shall submit the following documents. <br />Documents to Submit <br />Comments <br />Pg. 1-37 of RFP <br />Proposal Pricing Form <br />Must be filled out completely and signed <br />List of Independent Contractor(s), <br />Subcontractor(s) or Sub- <br />consultant(s), i.e. "Other Contractor" <br />Must be filled out completely <br />Reference List <br />Must be filled out completely <br />Sample Agreement for Services <br />Include all pages — please do not fill in blank <br />spaces. <br />Addenda, if any <br />Must be signed <br />C. SUBMITTAL <br />Firms wishing to have their proposal considered for this project shall submit the following <br />items below. <br />I. Firm Qualifications <br />A Statement of firm's qualifications applicable to this project, including the following: <br />a. State of California Contractor's License number and expiration date, C-27 & <br />C61/D-49 and any other applicable licenses <br />b. Names, qualifications and proposed duties of all staff to be assigned to this <br />project. The firm shall identify at least two (2) ISA Certified Arborists who will be <br />responsible for providing project management for the duration of the project as <br />well as a full-time English speaking Site Supervisor or Foreman, who is an ISA <br />Certified Arborist, capable of communicating with any City representative and <br />who is authorized to act on behalf of the firm. Personnel must be qualified and <br />trained in the tree maintenance industry. <br />c. List of staff qualifications including but not limited to: <br />i. All Certified Arborists employed by the firm. <br />ii. All Certified Treeworkers employed by the firm. <br />iii. All Certified Urban Foresters and/or Municipal Arborists employed by the <br />firm. <br />iv.
