6. Circulate the draft Relocation Plan for the required 30 -day public review and <br />comment period. <br />7. Incorporate any comments received and associated responses into the final <br />Relocation Plan to be approved by City Council. <br />Relocation Assistance Services — Permanent Displacement <br />OPC will provide the following assistance necessary for the effective relocation of <br />residential occupants to be permanently displaced (up to four households): <br />1. Inform displaced persons of available relocation assistance services and benefits <br />and explain the relocation process. <br />2. Provide displacees with on-going advisory assistance to minimize their hardship, <br />including referrals to, and coordination with, community service resources, public <br />housing, and other public services, as necessary. <br />3. Prepare and distribute the General Information Notice, Notice of Eligibility, Vacate <br />Notice, and other notices, as may be required. (All required notices will be <br />delivered in person or sent general and certified mail to establish proof of service.) <br />4. Request, and collect, written documentation of occupancy of the project units, <br />current rent, gross household income, legal presence status, utilities costs, age, <br />disabilities, and identification of the occupants. <br />5. Determine eligibility for, and proposed amount of, relocation benefits, including <br />moving payments and rental/down payment assistance payments. <br />6. Provide written referrals to replacement housing and assist displacees in locating <br />replacement housing (up to five sets). <br />7. Inspect replacement dwellings to determine if they meet "decent, safe, and <br />sanitary" requirements. <br />8. Prepare all applicable benefit claim forms, secure claimant's signatures on claim <br />forms, and submit claim forms to the City for processing and payment (up to three <br />RAP claims). <br />9. Monitor the move to the replacement site, as necessary. <br />10. Conduct vacate inspection and verify all personal property has been relocated from <br />the project site. <br />11. Deliver benefit checks and other appropriate payments to claimants. <br />12. Maintain necessary case documentation in compliance with applicable <br />requirements. <br />13. Upload relocation files electronically for hard copy and electronic file returns to the <br />City. <br />Project Management & General Consulting <br />Project management and planning will begin prior to the initiation of actual relocation <br />activities. It is critical that all members of the team are in agreement as to the City's <br />desired process and timeline working within the constraints of the applicable relocation <br />regulations. OPC's project management services will include: <br />1.
