<br />6.1A <br />Page 27 <br /> <br />light duty until the specific leave date, and shall continue <br />to furnish additional health statements from her physician <br />upon reasonable request. <br /> <br />9.6.2.3 Prior to the establishment of a specific date for return to <br />duty, the employee shall submit to the Personnel Officer a <br />notice of intention to return to duty, accompanied by her <br />physician's statement certifying that the employee is <br />medically qualified to assume regular duties and <br />responsibi lities. <br /> <br />9.6.2.4 The Personnel Officer or his/her designee may designate <br />the specific beginning and ending dates within the pay <br />period requested by the employee to meet the needs of the <br />employee and the City. <br /> <br />9.6.3 The employee on leave shall be returned to an equivalent position <br />within her classification. <br /> <br />9.6.4 A maternity leave, absent physical disability, is granted without pay <br />for the duration of the leave. The employee may elect to continue <br />medical and dental insurance coverage for up to one (1) year during <br />that leave at her own expense. <br /> <br />9.7 Leave for PreRnancv Disabilitv <br /> <br />9.7.1 Employees who are worki ng are entitled to use personal illness and <br />injury leave for disabilities caused or contributed to by pregnancy, <br />miscarriage, childbirth, and recovery therefrom on the same terms <br />and conditions governing leaves of absence for other illness or medical <br />disability. Such leave shall not be used for child care, child rearing, <br />or preparation for childbearing, but shall be limited to those <br />disabilities as set forth above. The length of such disability leave, <br />including the date on which the leave shall commence and the date on <br />which the duties are to be resumed, shall be determined by the <br />employee and the employee's physician; however, the Personnel <br />Officer may require a verification of the extent of disability through a <br />physical examination of the employee by a physician appointed by the <br />City at City expense. <br /> <br />9.7.2 Employees are entitled to leave without payor other benefits for <br />disabilities because of pregnancy, miscarriage, childbirth, or recovery <br />therefrom when sick leave has been exhausted. The date on which <br />the employee shall resume duties shall be determined by the <br />employee on leave and the employee's physician; however, the <br />Personnel Officer may require a verification of the extent of disability <br />through a physical examination of the employee by a physician <br />appointed by the City. <br /> <br />21 <br />
