<br />6.1C <br />Page 14 <br /> <br />SCHEDULED PREVENTIVE MAINTENANCE AGREEMENT <br /> <br />SCOPE <br />A scheduled preventative maintenance agreement that will consist of a complete 21 point mechanical and <br />electrical inspection of every ITT Flygt pump listed per our contract. This inspection will be on a 12 month <br />basis fonowed by a complete report of the findings~ The inspection is based upon agreement of both parties <br />involved in tbe services and terms stated in detail below. <br /> <br />TERMS <br />This agreement includes all labor and parts to complete the 21 point inspection listed on page 3. ADY <br />additional repairs not stated on tine 21-point check list required during the inspection would be performed <br />only by wI'itten authorization. ITT Flygt Standard Terms & Conditions, Payment Requirements (Net 30 <br />Days) pending credit approval by FRC, Standard Warranty applies. <br /> <br />ITT Flygt assumes no liability for loss of use or for any consequential damage with respect to the operation 01 <br />the pump~ equipment or accessories covered in this agreement. <br /> <br />It is the customers responsibility to bave the following at the job site when the lIT Flygt service technician(s <br /> <br />arrive: <br /> <br />A. Water supply for washing pump(s) <br />B. 120 Volt electric supply <br />C. Mechanic to assist in the inspection and learn proper <br />D. Electrician (if control Dot supplied by ITT Flygt) <br />E.
