Failure to adhere to this requirement will be considered a material breach of this Agreement and may lead to immediate termination of this Agreement and/or the termination of individual clients from the Diaper Bank program. b) Diaper donations to other agencies/entities. Community Partner hereby acknowledges and agrees it will not, under any circumstances, distribute diapers or other goods received from HAMO to other partner agencies, affiliate programs, or other entities without the express written permission of HAMO. Failure to adhere to this requirement will be considered a material breach of this Agreement and may lead to immediate termination of this Agreement. 8)Diaper storage. Community Partner will maintain secured storage of diapers in a clean and dry space and hereby agrees to an annual site visit by HAMO. In the event of diaper theft, Community Partner agrees to notify HAMO within 24 hours, by email or phone. 9)Diaper distributions to clients. Community Partner agrees to distribute diapers and other in-kind goods to its clients free of charge and with no obligation to pay, work, or attend religious services. 10)Client testimonials and data. Upon request by HAMO, Community Partner agrees, within 20 business days, to: a) Furnish HAMO with annual demographic data regarding its clients, b) Facilitate the bi-annual collection of client testimonials, c) Facilitate client video interviews or photo shoots, and d) Participate in the following surveys for data collection: i) An annual survey for program staff that work directly with clients whom benefit from the diaper program (the “Program Staff Survey”). Community Partner hereby agrees to provide HAMO with a staff email list, which will be used by HAMO solely for the purposes of completing the Program Staff Survey. Community Partner further acknowledges and agrees that the Program Staff Survey requires participation of at least 75% of program staff at Community Partner. ii) An annual survey for program administrators; and iii) An ongoing optional client survey distributed by program staff to clients receiving the diapers. 11)Marketing and Public Relations. a) HAMO may list Community Partner as a community partner agency on marketing collateral (digital or print) where community partner agencies are listed. b) Community Partner agrees to make at least one annual acknowledgement of the relationship with HAMO. This can be via social media, partner newsletter, and/or partner website. If Community Partner publicly acknowledges in kind donors (i.e., digital or print marketing collateral, annual reports), Community Partner agrees to include HAMO on its in-kind donor list.
