REV: 05-31-22 MI <br />J.Records of Work To Be Submitted <br />The Contractor will maintain a written record (made in the field at time of work <br />performed) of the items listed below. Records must be submitted to <br />SMCMVCD on the original field maps along with the Contractor’s work <br />summary sheet within two (2) working days after completion of each phase <br />of work, prior to billing. <br />If the inspection of a manhole or bait station is prevented (e.g. a paved over <br />manhole, flooding, tight manhole cover, etc.), the cause must be recorded. <br />The Contractor must inform the Public Works Director or designee and <br />SMCMVCD of this condition immediately and request their assistance to <br />remedy the situation so that the bait in the manhole may be inspected. <br />It is absolutely essential for the Contractor to keep an accurate record of all <br />pest activity (rodents and cockroaches). All evidence of rodent (gnawing on <br />the bait or rodent droppings) and/or cockroach activity found during the <br />inspection must be recorded on the field map and work summary sheet. <br />K.Required Data <br />i.Phase of work and dates performed <br />ii.Number of bait blocks/stations/manholes installed/sprayed with <br />the location of each designated on the maps provided <br />iii.Number of bait blocks/stations/manholes inspected and <br />accompanying field notes (e.g. percent consumption noted for each <br />block) <br />iv.Number of bait replacements / manhole resprays <br />v.Number of bait blocks/stations missing <br />vi.Number of bait blocks/stations re-installed <br />vii.(For Creek Program only) Number of bait stations removed in <br />October <br />L.Correction of Deficiencies <br />SMCMVCD will inspect each phase of work upon receipt of the Contractor’s <br />records (field maps and Contractor’s work summary sheet). In some <br />instances, every bait station will be checked or a portion of the stations will be <br />selected at random. Should any portion of the work not comply with these <br />specifications, the entire phase of work will be rejected by SMCMVCD until it <br />is corrected. <br />Therefore, it would benefit the Contractor should Contractor be notified by <br />SMCMVCD that certain deficiencies need correcting, to re-inspect the <br />identified deficiencies and expeditiously correct all deficiencies to insure that <br />no deficiencies exist. <br />During this re-inspection, to be made at the Contractor’s own expense, the <br />Contractor will be required to re-spray the pre-existing markings with blue <br />enamel paint to facilitate inspection of the Contractor’s re-inspection of all <br />ATTY/AGR.2022.127/Vector Pest Control (Dewey Services, Inc.)
