• Coordinate and provide oversight of the Inter -Agency Redwood City Homeless <br />Outreach Team. <br />• Incorporate peer outreach workers and individuals with lived experience as part of <br />the Homeless Outreach Team. <br />• Incorporate sufficient bilingual English/ Spanish staff to meet need of primary <br />Spanish speaking individuals. <br />• Create and monitor "by -name list" of unsheltered homeless individuals identified by <br />the Redwood City Homeless Outreach Team for outreach and engagement towards <br />housing and related services. <br />• Work closely with San Mateo County Human Services Agency (HSA) and the City <br />on the annual One Day Homeless Count, and other ad hoc initiatives to collect <br />information regarding people experiencing homelessness. <br />B. Provide outreach and engagement to unsheltered homeless individuals and families2 in <br />Redwood City: <br />• Provide regular and ongoing outreach and engagement to assigned areas in <br />Redwood City and to "by -name" list of individuals; <br />• Engage unsheltered homeless individuals and families to establish a pathway to <br />permanent housing and linkages to homeless system and mainstream services; <br />Provide rapid response to requests for homeless outreach staff to make contact and <br />provide support to unsheltered homeless individuals or families in Redwood City; <br />• Provide after hours and weekend coverage in Redwood City to address immediate <br />services or interventions for unsheltered homeless individuals and families that may <br />not be able to access mainstream services during business hours; <br />• Conduct additional outreach and assist unsheltered homeless individuals and <br />families with accessing shelter, provide water and other supplies, and connect <br />individuals to other resources during inclement weather activations and other <br />emergencies and <br />• Provide intensive coordinated outreach when an encampment clear out is planned. <br />2 "Family" refers to two or more individuals, including adult -only families and families with children, who <br />identify as a single household <br />REV: 05-16-24 MI <br />ATTY/AGR.2024/Amend.
