C. Connect unsheltered homeless individuals and families in Redwood City to housing and <br />cer�iirec• <br />• Provide case management services to support unsheltered homeless individuals and <br />families' transition to permanent housing; <br />• Provide housing services to support transition to permanent housing; <br />• Connect unsheltered homeless individuals to mental health support/services; and <br />• Connect unsheltered homeless individuals to substance disorder and recovery <br />support/services. <br />• Provide follow-up case management support as needed and feasible to individuals <br />on the by -name list who have transitioned into housing. <br />D. Provide resources for unsheltered homeless individuals and families in Redwood City: <br />• Provide or connect unsheltered homeless individuals and/ or families to hotel respite <br />resources administered by the City; <br />• Connect unsheltered individuals and/or families to housing subsidies administered <br />by the City; <br />• Provide supplies or other items to assist unsheltered homeless individuals or families <br />with meeting their basic needs and to build rapport (food, gift cards, socks, glasses, <br />etc.); <br />• Provide transportation assistance to assist unsheltered homeless individuals or <br />families with connecting to services. <br />E. Document and track contacts and services provided: <br />• Document and track all inquiries, responses, and case management plans in the San <br />Mateo County Clarity HMIS (Homeless Management Information System) in a timely <br />manner. <br />• Utilize additional, complimentary tools that assist with documentation and tracking <br />Objectives. <br />REV: 05-16-24 MI <br />ATTY/AGR.2024/Amend.
