REV: 01-17-24 MI <br />22.Consultant shall perform NASSCO MACP Level 1 manhole inspections for each <br />manhole in a given project area. In addition to the mandatory fields, Consultant shall <br />also include and complete the following fields on each inspection form: <br />•Owner <br />•Time <br />•Weather <br />•Drainage Area <br />•Location Details <br />Depending on the manhole condition, City may request that Consultant perform a <br />Level 2 manhole inspection for specific manholes. <br />23.Consultant shall review all deliverables prior to submission to the City for quality <br />assurance and quality control to ensure that all requirements are met and information <br />provided is accurate. <br />DELIVERABLES <br />Consultant shall submit the following deliverables to the City Public Works Services <br />Department contact electronically following achievement of project milestones or <br />tasks. <br />1. Deliverables for all pipeline inspections, both routine and ad-hoc, shall <br />include: <br />a. Pipeline Inspection Report – Consultant shall submit a quality- <br />controlled, coded pipeline assessment inspection report for each <br />sewer main segment. The report will include, at a minimum, all <br />requested PACP inspection form fields, observation details, condition <br />codes (including those for structural defects, operation and <br />maintenance, construction features, miscellaneous features), PACP <br />condition grades, PACP quick rating (structural, O&M, and overall), <br />segment grade scores (structural and O&M), overall pipe rating <br />(structural and O&M), and pipe rating index (structural and O&M). <br />b. Electronic Inspection Data – Consultant shall submit a quality- <br />controlled version of the complete electronic inspection data in a <br />NASSCO standard PACP exchange V7 database file (Microsoft <br />Access database, .mdb file extension) for direct entry into the City’s <br />WincanVX database. <br />ATTY/AGR.2024.007/APS Environmental, Inc.
