REV: 01-17-24 MI <br />c. Recording of Inspection Data – Consultant shall provide quality- <br />controlled video files of all inspection recordings either on an external <br />hard drive or available for download by City staff. <br />2. Deliverables for manhole inspections shall include: <br />a. Manhole Inspection Report – Consultant shall submit a quality- <br />controlled, coded manhole assessment inspection report for each <br />manhole. The report will include, at a minimum, all minimum and <br />requested MACP inspection form fields for a Level 1 inspection, <br />observation details, all condition codes, MACP quick rating (structural, <br />O&M, and overall), manhole grade scores (structural and O&M), <br />overall manhole rating (structural and O&M), and manhole rating <br />index (structural and O&M). <br />b. Electronic Inspection Data – If a Level 2 MACP inspection is requested <br />by City, Consultant shall submit a quality-controlled version of the <br />complete electronic inspection data in a NASSCO standard PACP <br />exchange V7 database file (Microsoft Access database, .mdb file <br />extension) for direct entry into the City’s WincanVX database. <br />c. Recording of Inspection Data – If a Level 2 MACP inspection is <br />requested, Consultant shall provide quality-controlled video files of all <br />inspection recordings either on an external hard drive or available for <br />download by City staff. <br />3. Additional deliverable for sanitary sewer basin inspections (Task 2): <br />a. Basin Summary Report – Consultant shall also provide a separate <br />basin summary report per basin in a Microsoft Excel format. The report <br />shall include the following: <br />i.All pipe segments with pipe segment reference, PACP <br />inspection form field information, and condition grades and <br />ratings per pipe segment. Examples of information that should <br />be included in the excel file are: inspection ID, pipe segment <br />reference, date of inspection, street name, upstream manhole <br />no. designation, downstream manhole no. designation, sewer <br />use, direction of flow, flow control, size of pipe, type of pipe, <br />shape of pipe, pipe joint length, total pipe length, actual pipe <br />length surveyed, pre-cleaning method use, weather condition, <br />condition grades and ratings, and likelihood of failure <br />calculation. <br />ii. All manhole inspections for the basin with MACP inspection <br />form field information and condition grades and ratings. <br />ATTY/AGR.2024.007/APS Environmental, Inc.
