Location of Services <br /> <br />Redwood City and North Fair Oaks, California <br /> <br />9. Deliverables <br /> <br />Final Project Closeout Report <br /> <br />The Grantee shall submit a Final Project Closeout Report to GO-LUCI at the end of the <br />Agreement or once the funds have been expended by the Grantee. <br /> <br />The Fiscal Sponsor and the Grantee are committed to ensuring GO-LUCI and all interested <br />parties are aware of the progress and outcomes achieved with this investment. <br /> <br />The report must be on the template provided in Exhibit E. <br /> <br />The final report summarizes the following information for this project: <br /> <br />o Use of funds for the project and remaining. <br />o Tasks and milestones completed for the project. <br />o Strengths and barriers of the project. <br />o Lessons learned from the project. <br />o Successes and outcomes of the completed project. <br /> <br />The report shall be submitted no later than thirty (30) days upon completion of the <br />project or end of the contract performance period, whichever is sooner. <br /> <br /> <br /> <br /> <br />THE REMAINDER OF THIS PAGE WAS INTENTIONALLY LEFT BLANK <br /> <br />
