REV: 02-04-25 LF <br />approve the proposed maintenance schedules before the work may commence. <br />All inspections, lubrication, adjustments, tests, cleaning, routine repairs, and other <br />preventative maintenance activities shall be performed in accordance with the <br />approved maintenance schedules. <br />The Contractor shall keep these work schedules on display in each equipment <br />room. Schedules posted shall be of the chart type, which shall be initialed by the <br />service person when each scheduled inspection is performed. <br />Contractor agrees to provide an activity log in a permanently bound journal having <br />pre-numbered pages and to keep the log current. The log is to be kept in the <br />machine room and available for review by City. The activity log is the property of <br />the City. The activity log will be complementary to the maintenance and lubrication <br />logs. Entries will identify who from the Contractor's organization was in the building, <br />during what period of time, and for what purpose (e.g. regular maintenance, regular <br />time call-back, overtime call-back, etc.). Equipment failures, passenger <br />entrapments or unusual maintenance situations are to be noted and require the <br />insertion of a brief explanatory entry. <br />A service/repair tag is to be filled out by the Contractor for each billable repair that <br />is not part of the contract. A copy of this shall be given to the City's Maintenance <br />Mechanic if present or it should be emailed to Facilities Maintenance by the next <br />business day. <br />9)Removal of Elevators from Service and Major Repairs <br />Contractor will coordinate and obtain approval from the Facilities Manager prior to <br />removal of elevators from service. <br />In the event major repair work becomes necessary, the Contractor will provide <br />separate repair personnel to accomplish the work to ensure that regular service <br />personnel continue with their normal preventive maintenance of the equipment. <br />10)Performance of Contractor <br />General: <br />Contractor shall ensure that only skilled and competent workers perform work <br />under the Agreement, and that those workers perform all work in a timely, neat, and <br />orderly manner. City may refuse to accept work or workers who do not meet these <br />standards. Contractor shall be responsible for any damage caused by Contractor's <br />operations and shall repair or replace, as directed by the City, said damage within <br />a reasonable time. <br />ATTY/AGR.2025/Amend. no.1/Star Elevator, Inc.
