REV: 01-16-24 MI <br />performance of this requirement would be extremely difficult or impractical to <br />determine in the event of such failure or delay, Contractor agrees that it shall be liable <br />for and pay to the City liquidated damages of One Hundred dollars ($100.00) for each <br />eight (8) hour delay beyond the allowed time. Contractor agrees to pay said liquidated <br />damages as herein provided, and in case the same are not paid, agrees that City may <br />deduct the amount thereof from monies due or that may become due to the Contractor. <br />All elevator service personnel responding to service requests or emergency calls shall <br />contact Facilities Staff upon arrival at the facility. The check in procedure shall be to <br />call the Facilities Help Line at (650) 780-7473 during business hours, or for after hour <br />emergencies to call Communications at (650) 780-7118 and request that they notify a <br />Facilities Maintenance staff person. <br />9) Schedules and Records <br />Contractors shall submit preventative maintenance work schedules to the Facilities <br />Manager. These work schedules shall be designed for each type of equipment to be <br />serviced, such as traction elevators or hydraulic elevators, and shall conform to the <br />manufacturer's recommended practice for the particular equipment concerned. The <br />schedules shall also show the type and frequency of service and lubrication proposed <br />by the Contractor for a one (1) year duration. The City will evaluate and approve the <br />proposed maintenance schedules before the work may commence. <br />All inspections, lubrication, adjustments, tests, cleaning, routine repairs, and other <br />preventative maintenance activities shall be performed in accordance with the <br />approved maintenance schedules. <br />The Contractor shall keep these work schedules on display in each equipment room. <br />Schedules posted shall be of the chart type, which shall be initialed by the service <br />person when each scheduled inspection is performed. <br />Contractor agrees to provide an activity log in a permanently bound journal having <br />pre-numbered pages and to keep the log current. The log is to be kept in the machine <br />room and available for review by City. The activity log is the property of the City. The <br />activity log will be complementary to the maintenance and lubrication logs. Entries will <br />identify who from the Contractor's organization was in the building, during what period <br />of time, and for what purpose (e.g. regular maintenance, regular time call-back, <br />overtime call-back, etc.). Equipment failures, passenger entrapments or unusual <br />maintenance situations are to be noted and require the insertion of a brief explanatory <br />entry. <br />A service/repair tag is to be filled out by the Contractor for each billable repair that is <br />not part of the contract. A copy of this shall be given to the City's Maintenance <br />Mechanic if present or it should be emailed to Facilities Maintenance by the next <br />business day. <br />ATTY/AGR.2024.006/Star Elevator, Inc.
