Sewer System Management Plan licensed for Redwood City use only Page 21 of 59 <br />Chain of Communications for Reporting and Responding to Spills <br />Step No. Staff Responsible Description of Activity <br />9 Utilities Field <br />Supervisor <br />Documentation: The Utilities Field Supervisor ensures that appropriate spills <br />are reported within two hours to Cal OES and the San Mateo County <br />Environmental Health Services Department2. The Utilities Field Supervisor also <br />ensures that all necessary steps and reporting have been completed, including <br />reports to CIWQS as required by the SWRCB. <br />10 Public Works <br />Superintendent <br />Documentation: The Public Works Superintendent verifies that all reporting <br />has been completed per City policy and regulatory requirements. Necessary <br />corrective actions are taken if reporting requirements have not been precisely <br />followed. <br />EFFECTIVENESS <br />The City utilizes the following Key Performance Indicators for measuring effectiveness of this Element: <br />1. Have there been any changes requiring updates to the Organizational Chart? <br />2. Have there been instances when a service call for a spill was not properly routed to response <br />personnel? <br />3. Were all spill response activities documented and forwarded to the LRO? <br />4. Have there been any changes in assigned responsibilities for implementing the SSMP? <br />5. Is there a process in place to ensure all contact information remains up to date? <br />IMPLEMENTATION PLAN/SCHEDULE <br />No. Plan Schedule Responsible Party <br /> PWS E&T <br />2.1 Review names, contact information and position <br />responsibilities.
