Attachment B <br />from HSFA will support the core nutritional and wellness services for older adults age 60+ and <br />over at the Fair Oaks Adult Activity Center in Redwood City. This model has been adjusted to <br />support the long-term impact of the health and wellness of vulnerable, primarily low-income <br />older adults. <br />Funding Requested $20,000. <br />Recommended Funding $10,000. <br />Samaritan House – Free Clinic of Redwood City. <br />The Samaritan House Free Clinic of Redwood City addresses health inequity for underserved, low- <br />income residents of southern San Mateo County. The clinic aims to increase access to health and <br />dental care for the medically uninsured to improve overall physical, social, and mental health <br />status; prevent disease and disability; detect and treat health conditions to improve quality of <br />life; and increase life expectancy, while alleviating demands placed on emergency rooms for <br />episodic primary care for the uninsured. <br />Funding Requested $20,000. <br /> Recommended Funding $20,000. <br />Service League of San Mateo County/Hope House Residential Program for Women & Infants. <br />The Service League's Hope House provides temporary shelter, food and other emergency and <br />life-sustaining services for an average of 40 individuals and their infants annually. Hope House is <br />fully licensed to administer a comprehensive, structured, residential substance use disorder <br />(SUD) and mental health treatment. Funding will help cover the costs of materials and supplies <br />needed to adequately serve this vulnerable population, which has been even more challenging <br />than usual. The experienced staff addresses the needs of individuals who have often been <br />involved in the criminal justice system and/or have come from shelters, local hospitals and detox <br />centers. This population is often homeless and has co-occurring disorders (i.e. substance use <br />disorder/mental health issues/health issues). <br /> <br />Funding Requested $20,000. <br />Recommended Funding $20,000. <br />Society of St. Vincent de Paul of San Mateo County- Peninsula Family Resource Center SVDP’s <br />Peninsula Family Resource Center (PFRC), part of our safety net services, is a homeless prevention <br />program that helps families and individuals throughout San Mateo County. SVDP provides <br />assistance with basic survival needs, such as food, rent and utility payments, transportation, <br />clothing, and furniture to working-poor families and individuals, the unemployed, and seniors <br />living on fixed-incomes- to help keep them housed and fed. <br />Funding Requested $20,000. <br />Recommended Funding $10,000. <br />9.C. - Page 11 of 27 <br />464
