Attachment B <br />Nuestra Casa <br />Nuestra Casa partners with Latino families to help them navigate the complicated systems that impact <br />their lives from immigration to social services to health care. Nuestra Casa develops educational offerings <br />in direct response to the needs of their members. This grant will allow Nuestra Casa to conduct 20 hours <br />per month of targeted Promotora outreach about affordable housing and emergency rental assistance <br />programs in low-income census tracts of Redwood City and support at least 20 community members with <br />researching, applying for, or renewing applications for affordable housing or emergency rental assistance <br />programs. <br />Funding Requested: $10,000. <br />Recommended Funding: $10,000. <br />One Life Counseling- <br />One Life Counseling Center will provide free and low-fee mental health counseling and family support <br />services to low-income children, adults, and families in Redwood City. The program reduces barriers to <br />care by offering rapid access (24-hour callbacks and first appointments within one week), multilingual <br />services, and culturally responsive treatment. <br /> Funding Requested $15,000. <br />Recommended Funding $11,000 <br />Peninsula Family Service – Fair Oaks Adult Activity Center – Meals and Wellness. <br />Peninsula Family Service strengthens the community by providing children, families, and older adults the <br />support and tools to realize their full potential and lead healthy, stable lives. Funding from HSFA will <br />support the core nutritional and wellness services for older adults age 60+ at the Fair Oaks Adult Activity <br />Center in Redwood City. This model has been adjusted to support the long-term impact of the health and <br />wellness of vulnerable, primarily low-income older adults. <br /> Funding Requested $20,000. <br />Recommended Funding $12,000. <br />Samaritan House – Free Clinic of Redwood City. <br />The Samaritan House Free Clinic of Redwood City addresses health inequity for underserved, low- <br />income residents of southern San Mateo County. The clinic aims to increase access to health and dental <br />care for the medically uninsured to improve overall physical, social, and mental health status; prevent <br />disease and disability; detect and treat health conditions to improve quality of life; and increase life <br />expectancy, while alleviating demands placed on emergency rooms for episodic primary care for the <br />uninsured. <br />Funding Requested $10,000. <br /> Recommended Funding $10,000. <br />Service League of San Mateo County/Hope House Residential Program for Women & Infants. The <br />Service League's Hope House provides temporary shelter, food and other emergency and life-sustaining <br />services for an average of 40 individuals and their infants annually. Hope House is fully licensed to <br />administer a comprehensive, structured, residential substance use disorder (SUD) and mental health <br />treatment. Funding will help cover the costs of materials and supplies needed to adequately serve this <br />vulnerable population, which has been even more challenging than usual. Experienced staff addresses <br />the needs of individuals who have often been involved in the criminal justice system and/or have come <br />from shelters, local hospitals and detox centers. The target population is often homeless and has co- <br />occurring disorders (i.e. substance use disorder/mental health issues/health issues). <br /> <br />Funding Requested $10,000. <br />Recommended Funding $10,000. <br />7.C. - Page 8 of 23 <br />38
