<br />6.18 <br />Page 12 <br /> <br />SCHEDULED PREVENTIVE MAINTENANCE AGREEMENT <br /> <br />SCOPE <br /> <br />A scheduled preventative maintenance agreement that will consist of n complete 21 point nu~chanicai and <br />electrical inspection of every JrTT Flygt pump listed per OUl' contract. This inspection win be on a _12_ <br />month basis foIlowedbya complete report of the findings. The inspection is based upon agl'cement of both <br />parties involved in the services and terms stated in detail below. Please note: This service proposal does not <br />include any shop repair or overhauls of the equipment to bein~pected. Sales tax on replacement pumps and <br />parts not included. <br /> <br />TERMS <br /> <br />Tbis agreement includes aU labor and parts to complete the 21 point inspection listed on page 3. Any <br />additional repairs not stated on tbe21..;point chec!{ list required .during tlte inspection would be performed <br />only by' written authorization. ITTWater & Wastcwatm' Flygt U.S.A.
