(0.1E -s <br />2.5 City <br />The City of Redwood City, California. <br />2.6 City Contribution <br />The non - elective contribution made by the City on behalf of a Participant which the City <br />allocates to fund the Participant's elected benefits under the Plan. The amount of the City <br />contribution will be determined annually by the City. <br />2.7 DCAP <br />The Dependent Care Assistance Plan attached hereto as Appendix D. <br />2.8 " Dependent " <br />The spouse and each unmarried child or stepchild of a Participant who is: <br />(a) Under age 19 or any greater age required by state law; <br />(b) Under age 23 and a full -time student who attends an accredited school; <br />(c) Physically or mentally disabled (as determined by the City), regardless of age; or <br />(d) Required to be covered under a Qualified Medical Support Order (QMCSO). <br />2.9 " Domestic Partner <br />An individual who shares a principal residence with an Eligible Employee, if the Eligible <br />Employee has provided any state registration form, affidavit or other document evidencing <br />domestic partnership status that may be required by the City or by the provider of a health <br />benefits plan offered by the City. A Participant who is legally married may not have a <br />Domestic Partner for purposes of the Plan. <br />2.10 Effective Dates <br />Premium Contribution Plan: October 1, 1989; Dependent Care Assistance Plan: October <br />1, 1989; and Medical Expense Reimbursement Plan: October 1, 1989. <br />2.11 Employee <br />Any individual who is treated by the City as a common -law employee. <br />
