PAYROLL RECORDS. The Contractor and subcontractors <br />shall submit weekly certified payrolls to the Housing and <br />Redevelopment Division, attn: Contract Compliance Liaison, P.O. <br />Box 391, Redwood City, CA 94064 or may deliver them to 884 Main <br />Street, Redwood City, within 7 days of the end of the reporting <br />period. <br /> <br /> State Payroll Form A-l-131 will be used. Payrolls must be <br />numbered consecutively and must contain the following <br />information: dates of the payroll period, name of each employee, <br />address and Social Security Number, work classification, day(s); <br />total hour(s)/week(s) including overtime, base rate of pay plus <br />fringe benefit(s) amount; withholding amounts and sources; and <br />the check number of the payment to each employee reported on the <br />payroll. <br /> <br /> Any employee or his or her authorized representative or <br />member of the public will be furnished a copy of the pertinent <br />payroll records by the Agency upon the payment of the copying <br />charge specified for payroll data in State law. The addresses <br />and social security numbers of employees will be masked or <br />deleted so as to prevent disclosure in copies furnished to the <br />public. Union representatives will be regarded as authorized <br />representatives for union employees and as members of the public <br />with regard to non-union employees. <br /> <br /> 5.
