6.4.A. - Page 5 <br /> ATTACHMENT1 <br /> Sec. 33A.7. APPLICATION FOR PERMIT <br /> A. An application for the types of permits authorized by this Article shall be made upon <br /> a form provided by the City and shall contain all of the following information: <br /> 1. The names, address, phone number, and e-mail address of each person and <br /> organization sponsoring the event or use. If an organization, the application shall <br /> describe the type of organization (i.e. whether not-for-profit) and contain the <br /> names, addresses, and phone numbers of the president or chairman thereof, and <br /> all other persons who: <br /> a. Have a position of management or control in such organization; and <br /> b. Are or will be engaged in organizing, promoting, controlling, managing or <br /> soliciting participation in such event. <br /> 2. The date, or dates, and beginning and ending hours of the event; <br /> 3. The location of the event; <br /> 4. The estimated number of persons who will participate; <br /> 5. The purpose of the event; <br /> 6. A description of any special parking arrangements or requirements for the event; <br /> 7. Whether any sound amplification equipment is proposed to be used, and if so, <br /> information describing such sound amplification equipment; <br /> 8. Whether or not charity, gratuity, or offerings will be solicited or accepted, or sales <br /> of food, beverages or other merchandise will occur; and <br /> 9. Such other information as requested by the Department. <br /> B. For applications for a block party the application shall also include: <br /> 1. A description of the portion of the block or intersection the block party will occupy <br /> and which is desired to be closed to traffic; and <br /> 2. The signatures of at least half of the residents fronting the location where the <br /> block party will be held. <br /> Sec. 33A.8. TIME OF FILING <br /> The application shall be filed not less than the following number of days prior to the <br /> event: <br /> A. For block parties: Ten (10) days prior to the date proposed for the block party. <br /> B.
